On any given morning in the villages that ring Lake Victoria, the first person a family calls in an emergency is rarely a doctor. More often than not, it is a neighbour — a community health worker trained by the Tanzania Rural Health Movement (TRHM) to act in the critical minutes before professional care arrives.

The heartbeat of the programme

Since 2018, TRHM has trained over 2,000 community members as certified first responders. These are fishermen, market traders, teachers, and mothers who volunteered to turn their everyday presence into a life-saving resource. Each one completes a hands-on curriculum built around the emergencies that actually occur in lakeside communities.

That focus on local reality is deliberate. A curriculum designed in a capital city will not serve a village reached by dugout canoe. So our training is written, practiced, and certified in Swahili, and rehearsed in the very conditions responders will face — dirt paths, market days, and the waterline itself.

What training looks like

Every new responder completes a structured programme covering the essentials of pre-hospital care:

Basic life support and safe rescue from water
Bleeding control and wound care
Recognition of stroke, heart attack, and diabetic emergencies
Safe transport and referral to the nearest facility
Community first aid education and prevention messaging

Training is delivered in short, practical sessions that fit around work and family life, so that participation never comes at the cost of a livelihood. Equipment is practical and durable — the same bags a responder will actually carry.

From classroom to community

The real education begins after certification. Responders are linked into a growing referral network that connects village first responders with health facilities, ambulances, and specialist partners such as Mwanza EMS. When a responder stabilises a patient on the lakeshore, they know exactly who to call and where to send them.

The road ahead

Two thousand responders is a milestone, not a finish line. With every new cohort, the distance between an emergency and a trained neighbour grows shorter. Our goal is a trained responder within reach of every community on the lake — so that no one waits alone for help to arrive.
